Medicaid expansion under the Affordable Care Act (ACA) has saved an estimated 27,400 lives between 2010 and 2022, according to a 2025 University of Chicago study. Other research indicates that in specific periods, such as 2014–2017, the expansion saved over 19,000 lives among older adults (ages 55–64

The Affordable Care Act (Obamacare) has significantly reduced uninsured rates for minority groups, with Hispanic, Black, and American Indian/Alaska Native (AIAN) populations experiencing the largest gains.

 

Experienced the largest gains, with uninsured rates for nonelderly Hispanics dropping from 32.6% in 2010 to 19.1% by 2023.

 The uninsured rate for Black adults fell from 24.4% in 2013 to 14.4% in 2018.

  • Lower Mortality: Medicaid expansion reduced the risk of death for low-income adults by 2.5% to 20%, according to a study by Becker Friedman Institute.

  • Age Groups: Younger adults accounted for nearly half of the total life-years saved.

  • Regional Differences: While 19,200 lives were saved in expansion states, 15,600 older adults died prematurely in states that chose not to expand the program.

  • Cost Effectiveness: The cost per life saved is estimated at $5.4 million.

Obamacare (ACA) has significantly improved substance abuse (SUD) coverage, providing over 32 million people with access to treatment and reducing mortality. By expanding Medicaid to 1.6 million people with SUD and mandating coverage for pre-existing conditions, it has enabled widespread access to medication-assisted treatment (MAT) and counseling, with studies indicating a 21% reduction in mortality for new, low-income enrollees

As of early 2026, 10 Republican-led states have not adopted the Affordable Care Act's Medicaid expansion, leaving roughly 1.6 million low-income individuals in a "coverage gap". These states are Alabama, Florida, Georgia, Kansas, Mississippi, South Carolina, Tennessee, Texas, Wisconsin, and Wyoming
